Disclosure regarding referral arrangement between Olympia Benefits Inc. and Solid Exempt Market Solutions Inc.

The unsigned client (the “Client” or “you”) hereby acknowledges to SEMSI that:

1. SEMSI is an Exempt Market Dealer, as applicable, and is not in the business of providing Private Health Services Plans (“PHSP”).
2. Your Advisor has, in his/her personal capacity, referred you to Olympia Benefits Inc. pursuant to an arrangement whereby Olympia Benefits Inc. may set-up a PHSP for you.
3. Any services provided to you as a result of this arrangement will not in any way be performed by or through SEMSI, and is an outside business activity (i.e. outside of SEMSI) by your Advisor.
4. Olympia Benefits Inc. will pay a portion of, any set-up fees and administration fees to your Advisor personally, the amount of which portion may vary depending upon the particular PHSP and other factors.
5. Your consent to the release by SEMSI of your personal contact information to Olympia Benefits Inc. for the purpose of this arrangement. Your failure to consent is not a condition of doing business with SEMSI and you are free to revoke your consent at any time

Trailer Commission Structure
Set-up Fee Commission and Subsequent Employee Registrant Commission are paid on a weekly basis. Claims Experience, Travel Insurance and Exceptional Expense Insurance Commission are paid on a monthly basis.
Agents who are paid by Direct Deposit must accumulate more than $30.00 in payable commission to receive payment.
Agents who are paid by cheque must accumulate more than $300.00 in payable commission to receive payment.
